This letter is to notify you that my practice, Taylor Family Clinic, will be closing in the next 30 days.  I have accepted a job out of state, and will no longer be practicing medicine on a regular basis in Texas.

I would like to give you the opportunity to obtain copies of your records or arrange for the transfer of your medical records to another physician.

As I use Practice Fusion for my Electronic Medical Records, and receive electronic faxes, your records are safely stored in an electronic format.

I started my career as a physician after caring for my father who died from colon cancer, my father-in-law who died from pancreatic cancer, and my grandmother who had Alzheimer's Dementia.  I learned that medicine is much more than pills, that quality of life is the most important aspect of health, and that health is a balance of mind, body, and spirit.  I do not endorse 5 minute doctor visits.  People should be treated with respect and need time to collaborate with a doctor.  Healing is a partnership, not fast food.  I also believe that insurance and paperwork should never interfere with the special relationship between a patient and a doctor.  The care I provide should be reasonable, affordable, confidential, and provide you with the knowledge and tools to help you make the best of your health within the context of your unique life, goals, and experiences.  I practice in La Grange and surrounding areas.  Contact me and let me know how I can serve you.

WE DO ACCEPT ASSIGNMENT BY MEDICARE WITH A 20% COPAY FOR APPROVED CHARGES.   We keep our fees reasonable, to help all folks who pay the same day.  Anyone with Medicaid is encouraged to use Tejas Healthcare, up the street from us, as we do not accept Medicaid at this time.

Appointment Request

We are glad to use Practice Fusion as our EMR. You will have access to Patient Fusion, the website that will allow you to have access to your medical records and secure HIPAA-compliant correspondence.  For any communications outside of Practice Fusion, as we are a small practice, trying to contain costs, we may use public email, phone, and voicemail services, such as G-mail and Google Voice, which may or may not be encrypted.  By providing your email address for communications, or leaving messages on our voicemail, you are consenting to this disclosure.
